Preston Coroner - James Adeley, Nicola Mundy (Doncaster) Coroners supervisor - John Crichton. Coroners Officer - Christine Milne. South Yorkshire Police - Lee Berry and David Wilson GMC, NMC, CQC. Not only did the hospital fail, so did the police, coroner, NMC, GMC they all covered up the death of Russell.
Wrightington Hospital failed in the care of Russell Buck Highton. The most important standard is that hospitals must tell patients and their families that a hospital mistake was made. By getting the hospital mistake out into the open, health care professionals will become aware of the problem and find ways to avoid making the same mistakes in the future. The new standards require that hospitals must work to prevent errors, design systems to ensure patient safety, and act on reports of medical mistakes.
Hospitals can be liable for the negligence of their employees including doctors, nurses and technicians. There are thousands of deaths every year in the UK which occur as a result of clinical negligence.
The Evidence is outstanding
The first document below is the Critical Incident Report Form this form was not at the inquest, it's the most vital document and somehow the coroner Nicola Mundy must have misplaced this document. It details the moving of Mr Highton's dead body to another hospital and Dr Wilkins lying to the son.
The second document shows the lawyer from Eversheds who represented the hospital at the inquest giving her summary of the inquest back to the hospital and even she can't believe that the coroner Nicola Munday withheld the Hospital Protocol from the inquest. This protocol clearly states that Mr Highton should have had TED Stockings on when back on the ward to prevent a blood clot. Why did the coroner Nicola Mundy withhold this vital document which exonerated the hospital?
The third document is the Internal Email sent around to the staff of Wrightington Hospital which has Doctor Boards, and Martyn Porters name listed. This email states that the coroner sent a message to the hospital telling them to take the bit about TED stockings off their Protocol so they don't get done in the future. Why is the coroner Nicola Munday doing this? Mr Highton died because he wasn't wearing TED Stockings which he should have been. She is now trying to protect the hospital? If you looked at the second document the coroner withheld the protocol and then tells the hospital to take it off.
Why didn't the coroner rule that the hospital had been negligent instead of covering Mr Highton's death up?
As you can see from the above documents Mr Highton's death was covered up. How many more times does the coroner cover up for the hospital? The sad part about all this is that family members aren't getting the truth about how their loved ones have died.
