For the past 18 months I have been trying to get answers to the terrible errors made at my fathers inquest on June 11th 2008 The coroner Nicola Mundy who has no medical training was told by James Adeley to conduct the inquest which was held at Ormskirk.

The mistakes made by the coroner are so bad that it really does show that she had no medical training. There are many mistakes this was one:

Andrew Foster is the chief executive of Wrightington Hospital the incompetence and lack of care at this hospital is astounding. My father had a previous DVT and they did absolutely nothing. There should have been many procedures put in place for my father these are just a few.

If a patient has had a previous DVT its so easy to care for the patient because the hospital, doctors and nurses have been given the information.

Doctor Board of Wrightington Hospital you killed my father. An orthopaedic surgeon needs to insist that their patients receive the most effective pre-operative and post-operative prophylaxis available. My father received nothing.

Doctor Board was told on numerous occasions that Russell Buck Highton needed attention to prevent a DVT yet nothing was done he never even went to see him on the ward. He never did his job this is a breach of duty and caused the death of Russell Buck Highton
